Insurance information and health questionnaire forms will be required on your first visit, so please come prepared to provide us with the necessary information so that we can be as thorough as possible; addressing any concerns that you may have along the way. This helps us get to know you a little better, and helps us make sure that we do the best we can for your specific case.
Your first visit with us will be about 90 minutes long. We will get to know each other, and we will become more acquainted with your dental goals, and what you are hoping to get from your visits with our dental office. This initial 90 minute visit does not include a cleaning, so please be sure to mention if you are in need of one so arrangements can be made to add this service to your visit. We will give you oral care suggestions, oral hygiene instructions, and provide you an in depth exam of your teeth. The exam includes: an oral cancer screening, dental charting, a joint and muscle check, a joint function check, x-rays, a periodontal screening, smile evaluation, and an intra-oral photo session of your teeth and their supporting structures.
To get the most out of your exam, communication is a must! If something in particular is bothering you, please be sure to let us know so that we can spend time checking the area. We will speak to you about any symptoms that we find, the diagnosis, treatments, and recommendations. A no charge consult may be required depending on the level of care required. We want to make sure your overall oral health is kept in great condition, and working together will help us to achieve this common goal.
You may bring a copy of our patient registration form, medical history form, and important office forms/policies with you to save time during check-in.
Payment Choices:
Of course, taking care of your teeth annually is the most affordable form of dental care. Keeping up with your annual exams can prevent more costly issues in the long run. We pride ourselves in taking the time to go over your individual case tooth by tooth. If you have had any substantial treatments in the past, you know what happens when teeth begin to decay. We keep our fees competitive, while cutting no corners when it comes to care. We do everything we can to ensure that our services are accessible to most, and cost depends on the complexity of your condition and the treatment required. Each patient is different, and a treatment plan will be designed specifically for you. For this reason, we cannot give a generalized estimated cost.
We will be happy to file your insurance claim for reimbursement. Dental insurance will help with some, or most, of your dental treatment if you carry it. Dental insurance has a deductible that varies by insurance provider, so your portion of payment will vary.

For you portion of payment, we accept cash or check, Visa, MasterCard, American Express, and Discover. For larger services, six-month interest free financing is available through Care Credit Personal Payment Plan. Apply online at carecredit.com, and if you are qualified, you will receive instant approval!
A five percent courtesy discount will be given for services that are paid for in advance, when the service is $3,000.00 or more. A $25.00 returned check fee will be charged for any checks that are returned by the bank.
